Build an ELITE ACTIVITY program or travel opportunity just for you or your loved one. Mix the elements of conventional wisdom developed by the big organizations (RecPartners may have many years experience working with these) with the spontaneity and individual needs of one family working with a professional bringing diverse competence and caring.

  • Note:  Private payment is available for people not utilizing Medicaid funding or for services not otherwise covered by Medicaid such certain entertainment or recreation or overnight travel. Contact your service provider or the State for clarification.

Hourly Rates to fit your budget

 

Punch Cards

10 time or 20 time Punch Cards are available which discount the hourly rate. These can be used over a one year period of time.  The number of punches needed will be noted by activity.

Monthly Day Program All Star Cards

One, two or three time a week monthly All Star access cards are available for a discounted hourly rate.

Special Events / Travel

Special Events are priced individually depending on the transportation needs, activity fees, length of time, food requirements and staffing needs.  Punch Cards may be applied to the fees as announced.  Overnight travel and custom trip costs to be arranged.

GETTING STARTED

If you already working with a CCB (Community Centered Board), contact your Resource Coordinator (RC) and ask to have Activity Options included in your IPO.  Your RC will issue a Prior Authorization Request (PAR) which gets thing started.

A RecPartner will contact you to continue the assessment process for you or your family member.  We will discover your goals, interests and abilities and determine together the best plan for you.

If you are new to the process and need to determine eligibility for government funding programs, check with your CCB or Department of Disabilities.

If you are working outside of a CCB, contact Activity Options directly and we will get you started by assigning you a RecPartner.  There are a few forms to fill out and the activity assessment and plan is underway!  Select your payment plan and you or your loved one are on the way.

Activity Options LLC has program approval from the Colorado Department of Public Health and Environment to provide services under HCBS-DD and HCBS-SLS: Day Habilitation-Supported Community Connection (SCC), Mentorship, Respite and Non-Medical Transportation and CES – Respite and SCC.
